East Granby Family Practice, LLC,  located at 13 Church Rd, East Granby CT, is a family practice medical group that has been serving the healthcare needs of our local communities since 1978. Our providers of care are Board Certified Family Physicians and Certified Nurse Practitioners. We provide all aspects of care for families and individuals including newborn, pediatric, adolescent, adult and geriatric care. We are proud of our long standing history within the community and of the up-to-date comprehensive care we offer while still giving personal care to each one of our patients in a cost-effective way. Our goal is to provide you and your family with the best healthcare possible to ensure a long and happy lifetime. EGFP is your advocate and goes the extra mile for patients as the patient-physician relationship is of paramount importance to this practice. EGFP is nationally recognized as a NCQA certified Level 3 Patient Centered Medical Home and has been the recipient of  national awards for delivery of high quality medical care.            

To provide the best service possible, patients are seen on an appointment basis only. For non-urgent and routine appointments please call in advance to insure a mutually convenient time with the provider of your choice. Describe your problem to the staff as best as possible so that we may ensure the proper amount of time will be scheduled.

Each day a number of appointments are left open intentionally to accommodate the acute needs of our patients. Two triage nurses are also available daily and are trained to assess the urgency of your problem.

Preventive health appointments or "physicals" are defined by the insurance companies as visits for comprehensive preventive medical care. These visits should be scheduled well in advance. During a complete physical exam we make sure you are up to date with your immunizations, cancer screenings and preventive health screenings. Do not wait for your physical to bring up medical problems. Please make a special appointment for any ongoing health problems, current or chronic, so that your physician will have enough time to give the problem the full attention it deserves.

At this time we have chosen not to use email as a way of patient communication. We feel it is more efficient for you to speak to one of the staff members directly. By using email there may be a chance your requests or concerns could go unattended. Your care is of the utmost importance to us.
